The annual selection of Community Heroes is one of the many ways CCA recognizes the grace and courage of children and teens battling serious illnesses.

The 24 Community Heroes selected each year participate in a professional photography session, and their portrait is featured on the CCA Wall of Courage, a traveling exhibit unveiling each year at the Celebration of Courage Family Festival (this year’s festival: September 6, 2008 at Portland’s World Trade Center). In addition, each Community Hero is presented with their special Community Hero Award.

You may nominate a child or teen who stands out to you or your staff as exhibiting the courage, spirit and grace of a “Hero.”

- Deadline for nominations is Monday, May 12.

- Nominees must be no older than 19 years old.

- Nominees must be facing serious medical challenges of any kind and may be terminal.  However, if terminal, they must be able to participate in a Children’s Cancer Association photo session in April or May, 2008 which can be arranged to meet the needs of the family.

- You may nominate as many children as you wish.

To nominate, send an email to CCA Music Rx® Director Emily Hoffmann at ehoffmann@e-cca.org. Include in your email the following information:

Child’s Name

Guardian’s name

Age

Gender

Also: Please tell us about the Hero you are nominating.

Hero Selection Process:  From the nominations received, a Hero Selection Committee will select a group of 24 nominees of varying diagnosis, age, ethnicity and cultural diversity.  CCA will then contact the selected heroes to be a part of this public recognition award. If the family chooses to participate, each child will be scheduled for a very brief photography sitting.

During the nomination process, we appreciate your support in adhering to HIPAA regulations.
